PROGRAMS WE SERVE
Adult Day Healthcare Center (ADHC)
The Fresno Center Adult Day Health Care Center (ADHC) is a community-based Adult Services (CBAS) program providing a variety of medical, therapeutic, rehabilitative, and recreational services.
CalAIM Community Supports (CS)
Services provided are substitute services or setting as medically appropriate and cost-effective alternatives and allow Members to obtain care in the least restrictive setting possible and to keep them in the community as medically appropriate.
CalAIM Enhanced Care Management (ECM)
The program offers a holistic, interdisciplinary approach to caring for Members with complex medical and social needs. It provides systematic coordination and comprehensive care management in a community-based, person-centered manner.
California Reducing Disparities Project (CRDP)
In collaboration with CRDP, The Fresno Center offers a mental health early intervention and prevention program aimed to reduce depression and anxiety, improve physical, psychological, social, and spiritual well-being, and increase awareness of mental health issues.
Cultural Based Access Navigation Support Services (CBANS)
The CBANS program operated by The Fresno Center uses Community Health Workers and Peer Support Specialists to provide culturally responsive navigation, improving access to behavioral health services for unserved and underserved populations by linking them with providers and systems of care.
Fresno HOPE (FHOPE)
Fresno HOPE (Health, Outreach, Prevention, Equity) coordinates agencies and community organizations to reduce health disparities in Fresno County by empowering Community Health Workers (CHWs) to provide culturally appropriate services, collect data, and enable families to participate in their health outcomes.
Living Well Center (LWC)
The Living Well Center offers culturally and linguistically qualified mental health practitioners for outpatient and intensive case management, aimed at individuals with serious emotional disturbances or mental illness within the SEA community, alongside a clinical supervision/training program for SEA students and graduates.
Promotora/Latino Navigator program
The Fresno Center’s Promotores/Navigator Latino, funded by Central Valley Regional Center, Inc (CVRC), offers language and general support services to individuals with developmental disabilities. This collaboration furthers CVRC's mission to assist individuals with disabilities and at-risk children in achieving their goals.
Southeast Asian Navigator Program
The Fresno Center’s Southeast Asian Navigator Program, funded by Central Valley Regional Center, Inc (CVRC), offers language and general support services to individuals with developmental disabilities. This collaboration furthers CVRC's mission to assist individuals with disabilities and at-risk children in achieving their goals.
